I hereby make application for membership and training in the Shaolin Institute of Chinese Martial Arts Association and upon acceptance, I pledge to obey all rules and regulations of this martial arts school, which are established for the purpose of keeping order and for the protection of the members from injury. I recognize that a risk is involved in the Martial Arts that requires my adherence to these rules, regulations, Sifu and any instructors.
I release and forever discharge the Shaolin Institute of Chinese Martial Arts Association and Morris Martial Arts, LLC, including its teachers, Instructors, agents, members, employees, representatives and all officials from all claims, legal actions, suits of law, or inequity whatsover which I, my heirs, executors, administrators as a result of any accident, injury, any physical, mental or emotional damages, losses sustained by me (or my child) as a result of being engaged in any physical activity, exercise, training practices. contest, clinic, tournaments, games, promotional examinations, demonstrations, travel to and from any of the forementioned, recreation and/ or any individual recognized as Instructor and/ or representing the Shaolin Institute of Chinese Martial Arts Association or Morris Martial Arts, LLC.
I further acknowledge and agree that any and all medical, dental, hospital and/ or other expenses which may be incurred by me or any person on my behalf in connection with any aforesaid accidents is my sole obligation and responsibility.
I waive any compensations and rights for any materials including but not limited to photographs, video tapes, DVDs, trophies and the like, used by the Shaolin Institute of Chinese Martial Arts Association or Morris Martial Arts, LLC for publication and/ or publicity.
